As mentioned, this particular puzzle in Project Zero: Mask of the Lunar Eclipse comes in Chapter 2. While playing as Misaki Asou, you'll end up on the second floor of the abandoned hospital, needing to input a code to open the patients' quarters and uncover the secrets of your past.
While in the 2F Nurse Room, you'll find a note saying that the code to the door is regularly changed, but that the children living in the hospital hide secrets around the quarters to figure it out.
You'll want to head back downstairs to the main entrance hall to the building, and walk into the small cubby hole underneath the big staircase. You'll pass an autosave point as you go in there, which is always handy.
Inside that room, search around using the flashlight. You'll soon stumble upon a scrawly hand-drawn image of the grandfather clock that resides in the main hall. Walk over to it and interact with it, at which point it'll glow blue.
Then, equip your Camera Obscura and grab a photograph of the base of the clock. This image will melt and reveal a four-digit number secretly emblazoned on the clock: 8395.
Once you have the number, it's as simple as heading back to the panel in the 2F Nurse Room and inputting that code. Be ready for a battle though, because a ghostly nurse will come out to battle you once the door's unlocked.
